It is
important to follow this process completely.
If the
Temporary License is not completely filled out, that
individual cannot race.
BEFORE
THE RACE:
§
Download the
Temporary License forms from for the USSA website.

•
Fill out the
Race Dates, Race Name and Location. This will help
speed up registration.
•
Make copies;
front to back, so you only have one piece of paper.

Download
Medical exemption release form for those that do not
have medical insurance.
•
Forms are in the Masters Race Packet
RACE
DAY:
§
Have the racer
complete the Temporary License form.
•
Ensure the
forms are completely filled and are legible – front
and back.
•
Ensure the
medical information is filled out completely.
•
Ensure the
racer prints their name and signs the form – on the
back.
§
Temporary
License payment.
•
The Temporary
License fee is $25.00 - for up to three consecutive days
of racing.
•
If paying by
check, the racer must make a separate check, payable to
USSA.
•
If paying by
credit card, check the credit card and ensure the
information on the Temporary License form is accurate.
•
Attach the
check or cash to the Temporary License form.
§
Foreign competitors with a valid Foreign
Competition License.
•
The foreign
competitors must complete the Temporary License form.
•
The fee for their Temporary License will
be paid by PNSA Masters.
AFTER
THE RACE:
§
Place all
Temporary License fees and paperwork in a 9 x 12
envelope.
•
Carolyn
Phillips (or her designate) will pick up the envelope
and mail it to Bill Skinner (USSA) the next day.
